举例:Dermatofibrosarcoma Protuberans
Dermatofibrosarcoma protuberans (DFSP) is a locally aggressive cutaneous tumor with low
to intermediate grade malignant potential. It has predilection for the trunk and proximal
extremities of young and middle-aged adults, with slight male predominance. DFSP presents
as a slow-growing, solitary or multiple, polypoid nodular lesion that ranges in size from
0.5 cm to 10 cm. Local recurrence is common (depending on the type of surgery used) and the
risk of metastases is rare (<0.5% of cases, usually to the lungs). Usually, metastatic disease is
preceded by multiple local recurrences and appears to be associated with fibrosarcomatous
transformation. The fibrosarcomatous variant can be seen in 10% to 15% of cases. It is
more aggressive, locally recurs in up to 75% of cases, particularly in acral lesions, and the
rate of metastasis can be seen in up to 20% of cases. DFSP is characterized by a reciprocal
translocation, t(17;22)(q22;q13) (COL1A1and PDGFBgenes) (223–227).
PATHOLOGy (FIGuRE 4-28A–E)
■ DFSP is a dermal neoplasm that almost always extends into the subcutis (the subcutis is
infiltrated in a lace-like pattern).
■ The tumor is composed of fairly uniform spindled cells with elongated nuclei showing
little or no pleomorphism and minimal pale cytoplasm.
■ The cells spindle cells are arranged in a storiform or cartwheel pattern (sometimes
centered on blood vessels).
■ In contrast with dermatofibroma, there are usually only small amounts of intermingled
collagen.
■ Thin-walled capillaries
